This is a responsible administrative and professional direct service work supervising the HUD Supportive Housing Program (SHP), including direct supervision of the case managers who provide supportive services. 

 

DESCRIPTION OF DUTIES:

Identify eligible individuals for Supportive Housing and Residential Services and verify their information.

Meet the public, give, and obtain routine and non-technical information, explain well defined rules and policies.

Complete referral information including name and a problem statement.

Collect rent from consumers, write receipts, balance reports, and turn in money each day.

Create letters and spreadsheets as needed.

Prepare vouchers monthly for appropriate facilities and turn them in on time.

Verify and Recertify SHP Consumers' information each year.

Refer the individual for each other services as may be appropriate.

Track and provide updates on assigned consumers in Nursing Homes.

Assume custodial responsibility for equipment and assure that equipment is maintained and operational.

Maintain central file on administrative matters.

Maintain suspense file of reports due, remind staff of due dates and assure timely submission.

Conduct monthly staff meetings for Supportive Housing staff.  Ensure minutes are kept and refer items for review to the attention of the Residential and Performance Improvement Directors.

Make routine reports to Residential Director on all matters of program and consumer activities.

Document as needed in the Electronic Health Record.

Maintain databases as required such as MICRS, HMIS, and other such systems.

Review and approve SUNR and ISP for all consumers in Supportive Housing programs.

Assist with correction of chart deficiencies noted in administrative/clinical reviews.

Provide administrative support to HUD projects to include written notices of delinquency, eviction, and recertification.

Track receipt of all incomes to include rent and utilities. 

Prepare invoices for utilities, rent and other necessary expenses.

Compile necessary reports for DHR, State Department of Mental Health and other interagency contract forms and forward to central office.

Transport consumers as needed.

Participate in training activities as required by Carastar Health to increase and enhance professional and therapeutic skills.

Complete reports required for client records as well as all forms and paperwork required by Carastar including Daily Service Reports and other forms as needed.

Supervise Supportive Housing Case Managers, ensuring they are performing all their duties as required.  Ensure case managers are meeting standards for productivity, documentation, provision of billable services, monitoring of apartments for cleanliness, supplies, etc.

Ensure case managers have completed a SUN Assessment Form on admission and every six months thereafter for each client utilizing information gained from the client or a significant other.

Ensure Case managers are determining resources that would meet the needs identified in the SUN Assessment by contacting provider sources, public and private, both in and out of the catchment area.

Monitor case manager's plans for the provision of community mental health services to the targeted individuals involved and review such plan not less than once every three months (Individualized Service Plan).

Monitor case managers' assistance in obtaining and coordinating social and maintenance services for the individual, including services relating to daily living activities, transportation services, habilitation and rehabilitation services, prevocational and vocational services, and housing services. 

Monitor case managers' assistance to the individual in obtaining income support services, including housing assistance, food stamps, and supplemental security income benefits.

Ensure case managers are completing the monthly unit inspection report, identify any areas of concern, and submit work orders or request for supplies and provisions as needed.

Conduct inspections of units in keeping with HQS standards. Complete required forms and documentation and notification to apartment managers.

Provide on-call coverage for Supportive Housing.

Perform other related duties as assigned by supervisor.

 

REQUIREMENTS:

Knowledge of the special needs and behavioral characteristics of the targeted, chronically mentally ill.

Knowledge of interviewing techniques and principles.

Knowledge of the use and side effects of psychotropic medication.

Knowledge of the legal and ethical issues relative to confidentiality of client records.

Knowledge of casework principles and methods related to counseling.

Ability and willingness to document activities and maintain records.

Ability and willingness to provide mental health services in non-traditional settings.

Complete and maintain Certification as a HUD Housing Quality Standards Inspector (HQS)

 

QUALIFICATIONS:

Bachelor's degree from a recognized college or university plus two years related experience or master's degree plus one year of related experience. Master's degree and some supervisory experience preferred. 

Experience with HUD preferred.

Must meet requirements for Approved Medicaid Provider status.

Must hold a valid driver's license and maintain a driving record that is acceptable to Carastar Health's insurance carrier.

Must maintain at least liability coverage on personal vehicles.
